1. What should I bring and wear to my first visit?
    Please bring your completed new patient information packet, insurance card, medical records from referring doctor(s), and all medication bottles. If possible, wear comfortable clothing or bring tennis shoes with you in case a treadmill stress test is ordered.
  2. How long can I expect my appointment to last?
    We are unable to give a specific time, due to the possibility of the doctor ordering tests the same day (if needed and available). We strive to get you taken care of as quickly as possible because we know your time is valuable.

    Consultation, EKG, any necessary tests that may need to be ordered after the consultation
  4. Do I have to have an appointment to get my lab work done?
    No appointment is needed. The lab opens at 7:30 a.m. Please be mindful that it does close around 11:00 on Fridays.
  1. Is there a fee to have certain paperwork (FMLA, disability papers, etc.) filled-out?
    Yes, the fee is $30 per form. Before requesting that any paperwork is filled-out it would be a good idea to confirm if there is or is not a fee.
  2. How much will I need to pay for my appointment?
    You will pay whatever your co-pay amount is for a specialist and any co-insurance or deductible. If you do not have insurance, you will be required to pay $250 at time of appointment and any remaining balance will be set-up on a payment plan.

    Yes, with our electronic medical records system your doctor will have complete access to all of your medical records.

  1. What is the turnaround time for my doctor to fill-out my paperwork?
    2 weeks (Keep in mind that there is a fee for some paperwork. Please refer to the questions under the billing section.).

    Please allow 2-3 days for the doctor to receive, read, and review the results.
